Please complete the adjacent form with your patient's details in order to register them with the Doctaly Assist remote patient monitoring service.

Upon submission, the patient or their carer will receive a WhatsApp message, welcoming them to the service and requesting their consent in order to proceed.

Based on the frequency that you recommend, the patient will receive prompts in WhatsApp to complete their assessment. Responses from each assessment will immediately be visible to the clinicians or healthcare professionals with whom the patient is registered.
